As a courtesy to our patients, we bill most insurance companies. Co-payments are expected at time of service. We estimate the co-payment due based on general information given by the insurance company, however, this is not always accurate. Because each insurance company has a varied way of describing services and payment percentages, our estimates, though carefully considered, may not reflect actual policy benefits. Insurance does not guarantee benefit payment until the services are completed and the claim is received, therefore we cannot guarantee our co-pay estimate, and additional fees may be owed by the patient. One statement will be sent showing the balance due. Payment is due upon receipt. A $25 re-billing charge will incur if a second statement is sent. All accounts over 60 days will be sent to a third party company for collections. Online payments are accepted 24 hours a day, 7 days a week.
Existing patients with no past due balance may choose this plan at time of service. A. balance due of minimum $300 and maximum $999 is divided into two payments. The first payment is due at the beginning of treatment and the second and final payment is due within 30 days from initial service. If the balance due is greater than $999, payment is divided into thirds. 1/3 is due at the beginning of treatment, 1/3 is due 30 days later, and the final 1/3 is due within 60 days from initial service. All debit or credit card charges will be processed on the date specified at initial agreement. There is a 3% finance charge. No changes are permitted once set up. If a change is absolutely unavoidable, a $50 fee will apply with a minimum of one week notice given. A $25 fee will be charged for any declined payments.
